Transition Tools, Policies and Plans for the Next Generation

Transition from a pediatric to an adult model of care for the adolescent and young adult patient within the same hemophilia treatment center (HTC), or from a pediatric HTC to an adult HTC, can be one of the most challenging times in the treatment relationship. This workshop will review the new Got Transition: National Center for Health Care Transition Improvement’s tools and highlight the key role social workers play in supporting patients and providers during this process. The participants will have an opportunity in small groups to develop a transition policy (the first of in the 6 elements required for a successful transition approach for a practice) and discuss their draft policy with other social workers during the session.
