For
the eleventh consecutive year, NHF is delighted to offer
its specially designed Youth & Adolescent
(Y&A) Program. Young people with bleeding disorders,
their nonaffected siblings, carriers, children whose parents
have bleeding disorders and children of presenters are
all welcome to sign up!
The program is scheduled to take place on:
• Friday, November 5, 8:00AM to 5:00PM
• Saturday, November 6, 8:00AM to 5:00PM
• Check-in will begin at 7:00AM on both days
Y&A program participants will be offered activities,
daytime meals (breakfast, lunch and snacks) and two field
trips. The program is specifically designed for children
and teenagers ages 4 to 17. A $30 fee per child is required
to offset the costs associated with this program.
POLICIES AND PROCEDURES
• There is no onsite registration for this program.
• You must complete and submit the preregistration
form to the NHF office by Friday, September 10, 2004.
• Be sure to complete the information about your child’s health and
dietary needs and sign the release of liability and field trip authorization
sections.
• A separate form must be submitted for each child.
• Children need to be signed in each morning by a parent/guardian between
7:00AM and 8:00AM.
• Children need to be signed out each evening by a parent/guardian before
5:00PM on Friday and Saturday
• Children are encouraged to check in and have their photos taken on Thursday
from 9:00AM to 5:00AM.
RULES OF CONDUCT
All participants must agree to the following rules of conduct:
• Confidentiality
• Respect and support one another
• Don’t interrupt
• Listen and learn from one another
• Clean up after yourself
• Respect other people’s property
• No “put downs”
• Never go off alone! (escorts will be provided)
• Observe local and national laws.
FRIDAY, NOVEMBER 5
Field Trip*
DALLAS WORLD AQUARIUM (AGES 4 TO
17)
Visit the salamanders! Watch the animals being fed! See walking batfish and
nearly invisible glass catfish! Visit 80,000 gallons of saltwater exhibits,
including sharks, stingrays and hundreds of reef fish all living in coral reef
ecosystem. There will be an educational program attached to the visit to learn
all about our friends in the water.
SATURDAY, NOVEMBER 6
Field Trip*
THE SCIENCE PLACE (AGES 4 TO 17)
Learn about toothbrushes through history. Watch the budgie parakeets and play
with electricity. Build and see impossible things through the art of optical
illusion. Oh, and don’t forget the dinosaurs!
