December 12, 2003 Dear Colleagues, On behalf of the Hemophilia Treatment Centers (HTC) at Alta Bates Medical Center and Children’s Hospital of Oakland, we invite you to attend the Region IX Hemophilia Program Annual Conference. Our gathering will be held Sunday, March 14 – Monday, March 15, 2004 at the Radisson Miyako Hotel, 1625 Post Street in San Francisco. Our HTCs, with assistance from the Hemophilia Foundation of Northern California, have planned an exciting program and are serving as local hosts. Bayer Health Care, Biological Division, is this year’s corporate sponsor, with additional support from the Centers for Disease Control and Prevention. January 30, 2004 is the deadline for conference registration, fees, and hotel room reservations. We have planned clinical and policy updates of interest to clinicians involved in the diagnosis and management of people with inherited bleeding and clotting disorders. The agenda is attached. Our conference is open to local clinicians, Health Department representatives, consumers, advocacy organizations, and industry representatives. Registration begins at 10:00 a.m. on Sunday March 14, 2004. We will start the conference differently this year by opening with a networking luncheon at noon. Formal presentations will follow from 1:30 – 5:00 p.m. Monday’s general sessions run from 8:30 am – noon. We’ll have lunch for the HTC and consumer subdisciplinary breakouts which last until 2:30. The Region IX Hemophilia Coordinating Committee meets from 2:45 – 5pm. Continuing Education: We are approaching Children’s Hospital Oakland, an accredited continuing medical education provider, to sponsor continuing medical education for physicians. The Region IX Hemophilia Program, an approved provider of continuing education by the California Board of Nursing and the California Board of Behavioral Sciences, Provider Numbers 12604 and 2142 respectively, designates this conference for 9 contact hours for nurses, marriage and family therapists and licensed clinical social workers. Conference Registration: Registration is required by January 30, 2004. Printable forms are available online at: www.hemophiliaregion9.org. Alternatively, you may complete and fax or e-mail the attached registration form to Stephanie Dansker at the Region IX Office [fax: 323-663-6896;sdansker@chla.usc.edu]. Registration Fee Payment: Registration is free for clinicians and consumers. A registration fee of $325 is required of pharmaceutical and home care industry personnel. There are no partial fees for partial attendance. Registration fees are due by January 30, 2004 and may be paid with cash, check, or money orders. Credit cards are not accepted. Please make checks payable to Childrens Hospital Los Angeles. Payment should be sent to Stephanie Dansker, Region IX Hemophilia Program, Childrens Hospital Los Angeles, 4650 Sunset Blvd., #54 Los Angeles, CA 90027. Lodging: Reserve your room by contacting the Radisson Miyako (415-922-3200 or 800-333-3333). In order to receive the special room rate, inform staff you are with the Hemophilia Conference." Room rates: $119.00 per night plus 14% tax has been negotiated for Sunday and Monday nights. All rooms are suites, and have a king, queen or 2 double beds. These rates can be extended three nights post conference based upon availability. The deadline for room reservations is January 30, 2004. Travel: The Radisson Miyako is located in downtown San Francisco, 14 miles from the San Francisco International Airport (SFO), on the corner of Post and Laguna. The hotel does not offer shuttle service, but door to door airport shuttles and taxi service is found on the airport’s upper level. One-way taxi service costs approximately $35 plus tip, shuttle service costs approximately $12 plus tip. Alternatively, BART is now linked to SFO. Upon arrival, take the blue line air tram to the BART station, take BART until you reach Montgomery Station, then take MUNI bus #38 on Market Street and get off at Geary & Laguna; the Hotel is one block away. Parking: There is very limited valet parking for $20.00 night. the Japan Center Garage across from the hotel is available at $13.00 for overnight parking, and at $9.00 for the early bird rate (in between 6 – 9 am and out before 7 pm). Attractions: The Radisson Miyako is located in Lower Pacific Heights, just minutes from Fisherman’s Warf, Union Square and other major attractions.
