Guidelines

  • Applicants are not required to be patients of FOSRX/FAST, but must have an eligible diagnosis (family members of diagnosed person are not eligible) and be a resident of the U.S.
  • Applications will be reviewed by committee. Finalists will be required to submit a recent transcript and will be interviewed by the scholarship committee via phone interview. Recipients will be notified after final deliberations.  Checks will immediately be sent out after all scholarship requirements have been met.
  • All scholarship funds must be used for furthering the education of the recipients through a private or public college/university within the U.S.
  • We are looking for applicants who are already accepted to a college/university, and who are willing and able to attend a community event with us as a representative of our scholarship program at some point during the upcoming school year. This is a new requirement this year, so please take note before proceeding.

*By submitting a photograph, and if chosen to be a scholarship recipient, you give permission and rights of that image to FOSRX/FAST for use in promotional and marketing materials. Winners will be required to submit additional photos upon request with scholarship check.

Platinum: $5,000

Eligibility:

  • Must have been diagnosed with a bleeding disorder.
  • Must have 3.8 GPA or higher to apply.
  • Preference given to those interested in science, healthcare, medical, and pharmacy fields.
  • Looking for outstanding students with a variety of talents and extracurricular interests.

Gold: $2,500

Eligibility:

  • Must have been diagnosed with an immunodeficiency (CIDP/IVIg patients).
  • Must have 3.5 GPA or higher to apply.
  • Preference given to those interested in science, healthcare, medical, and pharmacy fields.
  • Looking for outstanding students with a variety of talents and extracurricular interests.

Silver: $1,000

Eligibility:

  • Must have been diagnosed with one of the following conditions: Rheumatoid Arthritis, Psoriasis, Multiple Sclerosis, Crohn’s Disease, Hepatitis C, or have received an organ transplant.
  • Must have a 3.5 GPA or higher to apply.
  • Looking for outstanding students with a variety of talents and extracurricular interests.
